ATTENTION: Please Read the directions!

Only data from Authorized IJAS Regional Sponsors will be processed. To insure this process an IJAS 11 password is now required. Contact your Region 11 Chair to get the password. If you are not an authorized IJAS Sponsor your submission will be ignored. In order to minimize the possibility of the loss of data only 1 project or paper can be submitted per form. To enter additional projects/papers, just fill out and submit multiple forms until all your students have been entered.
You may use this form to register to register both Projects and Papers for IJAS Region 11 Science Fair
Fill out a NEW form for each project/Paper, you should REPEAT this process as many times as needed for your school. i.e. if you have 8 projects you will have to fill out and submit this form 8 times, once for each project/paper. After submitting the form you may use your WWW browser BACK arrow/button to help you recall sponsor/school information that is repeated on each form. This way you need change only the student/project information and resubmitting the form saving you retyping information over multiple times.
When you have completed filling out this form you must scroll to the bottom of this form and select the Submit Button or else your information will not be sent.

Student/Project/Paper Information Remember to Check for Correct Spelling of Student Names and Project Titles. This is the information that is used on the Award Certificates!!

2nd Student Name:	First Middle Last If only one student is on the project leave 2nd entry blank
Title:
Type:	Is this a Project or a Paper? Use your mouse to select from this menu.
IJAS Category:	Use your mouse to select from this menu for a category
Grade:	Use your mouse to select grade level
Electricity:	Use your mouse to select Yes or No

After submitting this form you should use the Print option of your WWW Browser to print a copy out of the Next WWW Page you see on your screen. This will be your hard copy. You should also use the Save Option to keep a copy of that output as your electronic record should the original become lost/corrupted over the Internet. Please note, you should NOT print/save a copy of this page since it is an electronic form. If you do all the fields which you have typed in will be blanks. This is not an error on your part or with your computer, that is just how electronic forms work!
